Responsibilities of the RN Field Supervisor:

• Coordinate with colleagues to assess, plan, implement, and evaluate patient care plans.
• Train caregivers in Home Health Aide skills, both in person and at client settings.
• Develop and implement personalized Care Plans for each client, ensuring client and family approval.
• Interact effectively and professionally with clients and their support networks.
• Complete Client Service Agreement packets for each new client and manage initial service fees.
• Review, update, and sign off on Care Plans with clients and designated family members.
• Ensure caregivers are compliant with the Care Plan and provide the highest quality care.
• Document changes in patient conditions and maintain accurate records of care provided.

Qualifications of the RN Field Supervisor:

• Current Registered Nurse (RN) license in the state of employment.
• Registered Nurse with 2 year’s experience in home care or aide supervisions is preferred.
• Excellent interpersonal and client care skills.
• Valid driver’s license.
